Requesting external services

Gosford East Public School provides access to external providers.  Providers do not have an automatic right of access to a school. Any request for access to a student by an external provider funded by the NDIS (or other scheme that provides funding to individuals) must come from the student’s parent or carer in writing.

Providers, please read the  fact sheet for providers on mandatory child protection roles and responsibilities  from the Department of Education.

Parents: Please read the  Parent and Carer Fact Sheet  from the Department of Education then complete the Parent/Carer Request for External Provider Form.

To meet the requirements of the school curriculum, and in the best interest of the student and the school, external providers will be able to attend Gosford East Public School once a time and day is discussed with the teacher. It remains the parent's responsibility to advise providers if their child will be absent from school or if there is a school activity on the day of support.

Mainstream students: to reduce disruption to the class and individual student's literacy and numeracy, we ask external providers to come from 8.30am - 9am or after 12.30pm . Recess and lunch times are not preferred for support, as these breaks are important opportunities for the student to build friendships and develop social skills.

Support Class students: the above time restrictions do not apply. Please check with the class teacher via the office email.

There is a  External Provider Checklist  with the documents and requirements for external providers.  Please read this checklist in full and ensure each and every check/requirement is met before submitting any documents to the school. These requirements/checks are from the Department of Education. The External Provider Engagement Agreement has changed from last year. Please use the new External Provider Engagement Agreement.  NOTE: Attachment B Form 1 is to be signed by the student's parent.

Please submit all paperwork and attachments in one email with the day and time you are hoping to visit the student.

You will need to receive an email or phone call of approval prior to starting sessions.

Therapists must read:

Therapists must complete and return:

  1. An External Provider Agreement
  2. Current child protection training  -  must be done every year

Therapists must provide evidence of:

  1. Public Liability insurance for at least 20 million dollars
  2. Personal indemnity insurance for at least 2 million dollars
  3. Workers compensation or insurance (not required for sole traders – however, evidence of sole trader status must be provided).
  4. Completed child protection training- for current year
  5. Completed Anaphylaxis training
  6. Photo ID

This process is to be completed at the beginning of each school year.
